There is a print out of the form to use for the initial evaluation, progress notes and home programs. Also included is a description of how to use the video for home programs.
A single copy of the video, form and instruction sheet sells for $40.00. Additional copies of the video sell for $20.00 each for up to 20 copies and $15.00 each for over 20 copies.
